Sermorelin binds the GHRH receptor on anterior-pituitary somatotroph cells, coupling primarily through Gs and adenylyl cyclase to raise cAMP, with associated MAPK signaling and calcium-facilitated vesicle fusion.
Because it stimulates natural pulsatile secretion and remains subject to somatostatin feedback, it is characterized as highly somatotroph-selective in vitro.
